Important Notes

- Have your agent number and/or AgentLink User ID# handy
- Answer enrollment questions, using enrollment checklist.
- Inform client of follow-up verification call, to be generated automatically 3 hrs after completion of enrollment.
- Provide client with copy of Summary of Benefits and Temporary ID Card. Write the subscriber number on the Temporary ID Card.
- Temporary ID cards for new enrollees can be downloaded from AgentLink or ordered from Donnelley.
- Submit any other applicable paperwork (PAC form, Power of Attorney Documentation or Scope of Appointment form) through DirectFAX or mail.
- Clearly mark the subscriber ID number on each document.
- DO NOT SUBMIT a completed paper enrollment form after telephone enrollment (will delay processing).
- ExpressLine is THE preferred enrollment method for MA/PDP plans.
EASILY Enroll Members Who are Losing PFFS Plans in 2010
- Universal wants to make it easy for you to move your clients who are losing their Private Fee for Service with another Carrier.
- Universal American will provide you with "co-op letters" that can be mailed to these clients (these will be available October 1st, 2009).
- You will be able to enroll clients in one of the Universal American plans over the phone - no paperwork required.

Submitting PFFS/PPO/HMO Business
Switching Plans
2009 Telephone Verification Call.
- Verification procedures for PFFS
- Agent CANNOT be present when the customer completes the verification call.
- The agent should call the verification hotline at (800) 260-5769 to schedule the verification call when they are with the customer and completing the enrollment form.
- Schedule the verification call and write the appointment tracking number in the upper right hand corner of the front page of the enrollment form and in the box on the acknowledgement form.
- Commissions will be advanced after a properly completed enrollment form is processed by Today's Options and submitted to CMS.
- The procedure for PFFS is essentially the same as for the 2008 selling season.
- Verification procedures for HMO and PPO
- The agent can be present when the customer completes the verification call.
- For HMO enrollments, the agent should call the verification hotline at (866) 866-7391.

For PPO enrollments, the agent should call the verification hotline at (800) 260-5769.

You will be assigned a call tracking number by the operator on the verification hotline after the call is completed. Write the appointment tracking number in the upper right hand corner on the front page of the enrollment form and in the box on the acknowledgement form.

If the verification call can't be completed at the time the enrollment form is taken, then schedule the verification call and write the appointment tracking number in the upper right hand corner on the front page of the enrollment form and in the box on the acknowledgement form.

Commissions will be advanced after a properly completed enrollment form is verified, processed by us and submitted to CMS.

If the verification call can't be completed and the enrollment form is still processed by us and submitted to CMS, the agent will be paid on an as earned basis and no advance commissions will be processed.

The procedures for HMO are essentially the same as for the 2008 selling season. (PPO is new for 2009.)
