- In addition to the enrollment form, Geisinger is requiring a copy of the Sales Appointment Confirmation Form. If you do not complete and submit this form along with the enrollment form, the application will be marked incomplete and commission will not be paid! You should keep the original of this form in your client's file for 10 years.
How to submit business
- Enrollment forms should be IMMEDIATELY Faxed (preferred) or mailed.
- Geisinger applications (and Confirmation Form) should be mailed directly to Geisinger in the envelope provided in the enrollment kit (Geisinger Health Plan, PO Box 900, Danville, PA 17821-9989). Also, be SURE that your agent number, name and signature appear on the ORIGINAL COPY. Since the application is a triple carbonless copy, this information will not come through if you print this information on the last copy (it is only carbonless one way).
- You may also fax your enrollment application and Sales Appointment Confirmation Form to (570) 271-5970. YOU DO NOT NEED TO MAIL THE ORIGINALS!
- If you enroll a client on the Medicare.gov website, you will need to immediately call Geisinger's Call Center and let them know that you assisted your client in enrolling electronically. The number for the call center is 800-631-1656
Other Instructions:
- All apps must be completed with the broker signature and corresponding broker ID (without the broker ID number it can delay processing)
- Faxed applications must be complete (all required pages must be present) when faxed. MSA apps must have the signature form for Mellon complete to get the account established. Applications that are incomplete or are illegible due to the poor quality of the fax from the sender's machine may be pended due to volume.
- When an application is faxed and then mailed, please make sure to clearly note on the mailed application that the application was previously faxed and the date it was faxed.
- Apps can also be scanned and e-mailed to mtsholder@thehealthplan.com (always CC rbuggy@thehealthplan.com ).
Pending Approval: Online Enrollment Process (DO NOT USE THIS UNTIL APPROVED!!)
- Broker enrolls client through the Online Enrollment Center on Medicare.gov. At the end of the election (after they press submit) they obtain a confirmation number for the election made on line.
- Broker calls the BSU to connect themselves with the application. Broker will give the BSU rep the following information:
- Information required
- Broker Name
- Broker ID,
- Client's name (as it appeared on the application),
- Date the application was submitted and,
- Most importantly, the confirmation number issued by Medicare for the election.
3. Broker ID is placed on the application for them to be paid.
