Order 2010 Supplies Online

In order to access the ordering system, you need to have passed the product tests and have an Agent Writing Number.

• Each agent is provided a monthly allocation of kits for each CCP product that they are eligible to sell.
  • If you order more kits than what has been allocated, you order will be pended for review.
    • You will receive an email that communicates the final determination of your request.
• Starting October 1st, log on to the Coventry Broker Portal and click on the "Order Sales Supplies" link on the left navigation panel to requiest your CCP  sales kits. 
• To access handy instructions that guide you through the order process click on Sales Supply Order Instructions.

Health America Supply Orders

• Central PA - Adams, Berks, Carbon, Centre, Columbia, Cumberland, Dauphin, Lackawanna, Lancaster, Bebanon, Lehigh, Luzerne, Lycoming, Mifflin, Monroe, Montour, Northampton, Perry, Schuylkill, Sullivan, Wyoming and York
  • Supply Order Form for Central PA (Also available in Resource Center at Agent Gateway!)
  • Fax Form to 1-866-669-2430 or e-mail to faxmedmktpap@cvty.com
  • Quantity Limits:  Roughly 20-25, however, higher amounts can be placed by General Agencies.  Up to 5 Provider Directories and up to 5 additional Summaries of Benefits
  • NO PO BOXES, they will be shipped UPS, so they need a Physicial address. 
  • Kit includes:  Formulary, (2) Enrollment Forms and a Summary of Benefits.  Provider Directories are NOT INCLUDED and must be ordered seperately.  Additional Summaries of Benefits can be ordered.
    • If you would like extra Enrollment forms, note this in the "Special Instructions" section.
• Western PA and Ohio - Allegheny, Armstrong, Beaver, Bedford, Butler, Clearfield, Erie, Fayette, Greene, Lawrence, Mercer, Washington and Westmoreland PA.  Columbiana, Mahoning and Trumbull, OH.

  • Ordering Process (HMO/PPO Products)
    • Step One:  Order the number of kits needed in space near top of form.  Kit includes ONLY (2) enrollment forms and formulary.  Kits are Generic with NO SUMMARIES OF BENEFITS!
    • Step Two:  Order the number of Provider Directories you will need.
    • Step Three:  Order the specific summaries of benefits you will need to match with your kit order.
    • Step Four:  If you need extra Enrollment forms, note this in the "Special Instructions" section.
  • Ordering Process (SNP Products) - Kits will include the Summary of Benefits.  You will just need to order Provider Directories, if necessary.
