• Click here for Provider Lookup
• Select the Plan from the Dropdown Menu and Click the Yellow Arrow button
• Click on "Find a Doctor" or "Find a Hospital", etc. and follow instructions

"Hard Copy" Provider Directories

• You can download a Provider Directory from the Agent Portal
  • Log into Broker Portal
  • Select the Marketing and Advertising Materials on the left Bar
  • Click on "Click Here"
  • Page Down to Enrollment Kit Components and click on "2010 Enrollment Kit Components
  • Page Down until you find the Directory for the Plan you are looking for.
