1. Client's Name is Optional
2. Select Zip Code
3. Select Age at time of Application
4. Select Gender
5. Select Rate Class (Tobacco Use = Tobacco, No Tobacco Use = Preferred)
6. Check the Level and/or Graded box
7. Enter the Face Amount (Do not put in a $ sign or any commas)
8. Agree to Terms and Conditions
9. Click Submit
