4. Enter Rate Class (Tobacco Use in past 5 years = Tobacco, No Tobacco Use in 5 years = Preferred)
5. Check the Cancer Insurance Check box
6. Select a face amount from the Drop Down
7. Agree to Terms and Conditions
8. Click Submit
