How to Create a New Service Request
- Select the Topic from the Drop Down Menu. Can be any of the following:
- Claims
- Commission
- Enrollment
- Licensing
- Marketing
- Other
- Question
- Supplies
- Enter the Subject - Include the Company Name for which this pertains. (Ex. Aetna Claims Issue)
- Enter the Description - Add additional detail, including any information you believe we will need to help you resolve this issue, such as the client name, the client policy number, pertinent dates, what action you have taken, etc. Example: Mary Smith (policy number 123) had a claim rejected (Date of Service is 8/11/2008) for Dr. Jones. Aetna said this doctor is out of network, but doctor is showing up in the Provider Guide.
